P URPOSE The VCOM Policy on the Collection and Review of Employee Driving Records has been implemented in response to requirements promulgated by VCOM’s automotive insurance carrier. The continuance of VCOM’s automobile insurance coverage is dependent upon the safe and legal driving practices of its employees. 2. VCOM D RIVING R ECORD Q UESTIONNAIRE To meet the insurance requirements, VCOM asks all employees to complete an annual questionnaire about their driving record. All full-time and part-time salaried employees will be asked to complete a driving questionnaire each year at the time of open enrollment. The questionnaire will ask employees to Based on responses to the VCOM Driving Record Questionnaire, employees that frequently operate motor vehicles for VCOM’s business travel may be requested to grant VCOM permission to obtain a copy of their motor vehicle driving record. Failure to answer questions honestly and fully may result in termination. The employee will sign an authorization form, allowing VCOM to obtain their motor vehicle driving records for the past 36 months. In addition, for those who are identified by supervisors as frequent drivers for VCOM business, VCOM may periodically request a background check.
