Appendix C Faculty Recruitment Reference Check Form

They purpose of the reference check is to verify information provided by an applicant and obtain information about previous job performance. Not all former employees will answer all the questions listed below, however, please ask all the questions and document if an answer is not provided by inserting “did not answer” for questions without a response from the former employer. References should be checked from all employers during the past 10 years, however, please use your judgment and not contact a former employer if you feel the previous position is irrelevant such as working in fast food restaurant while they were attending school.
