Attachment A Edward Via College of Osteopathic Medicine Waiver of Liability for Driving Personal Vehicles on College Business

I release and hold VCOM harmless from all liability, if I am involved in an automobile accident, while driving my personal vehicle on college business. Also, I will pay any and all claims, costs and liabilities

resulting from automobile accidents involving other parties. To minimize my liability, I have been advised I should have: (a)

A valid driver’s license issued by the State in which I reside.

(b) Personal automobile insurance coverage, at a minimum, of $100,000/$300,000 for bodily injury and $100,000 for property damage, or $300,000 combined single limit. (c) No moving traffic violations by the Department of Motor Vehicles that include reckless driving; driving under the influence; habitual offenses; and driving on a suspended, restricted, or revoked license.
