3.4.1 The Application Process for Initial Appointment A candidate seeking initial faculty appointment assumes the responsibility for preparing an application packet that includes: • Cover letter formally requesting consideration for initial appointment that specifies the academic or clinical rank sought (Assistant Instructor, Instructor, Assistant Professor, Associate Professor, Professor) • Current Curriculum Vitae (CV); • Proof of degree; o For the purpose of accreditation and state licensure, proof of degree of an academic track faculty member whose degree is less than doctoral, must be accompanied by a letter from the Campus Dean that: Justifies the faculty member’s professional and scholarly achievements and/or demonstrated competences in the discipline of hire. o For the purpose of accreditation and state licensure, proof of degree of an academic track faculty member whose doctoral degree comes from outside the U.S., must be accompanied by a letter from the Campus Dean that: Justifies the faculty member’s professional and scholarly achievements and/or demonstrated competences in the discipline of hire, and States the findings of the transcript evaluation service as to the equivalency and verification process of the degree (a copy of the report from the evaluation service must be included). o If a residency certificate is not available, proof of initial board certification and personal verification of having completed post-graduate training will suffice. • Proof of a current medical license in the state in which they practice with no sanctions for physicians; o See section 3.1 for specific requirements regarding state medical licensure for Clinical Department Chairs and the Chair for OMM. • Proof of board certification or board eligibility for physicians; o Proof of board certification may be in the form of: A copy of the certificate; or A verification report obtained from the certifying board. o Proof of board eligibility may be in the form of: Proof of post-graduate training and proof of current state medical licensure when initial board certification has not been granted; or Proof of initial board certification and current state medical licensure. o See section 3.1 for specific requirements regarding board certification for Clinical Department Chairs and the Chair for OMM. • Proof of any additional training certificates if applicable; • Proof of prior teaching experience in the classroom or clinical setting (if any) to include: accomplishment in the area of teaching, evaluations of teaching • Proof of post-graduate training for physicians may be in the form of a: o Residency certificate; or
