form of coverage to meet the requirement. Important: Students on Medicaid cannot hold more than one policy. It is imperative that students get information at orientation and take the appropriate steps if they are on Medicaid to avoid losing their benefits. If medical students provide their own health insurance, the policy must meet the minimum coverage levels. These include: • Insurance company must be based and operated in the United States • Must provide in-network coverage in the state where the student resides while attending VOCM • Deductible Maximum: $2,500 • Lifetime Maximum (suggested): $1,000,000 • Co-Pay: Students should seek no higher than 30% co-pay with insurance covering at least 70% up to $5,000 maximum and 100% payment by insurance after $5,000 is met. VCOM does not waive student health insurance requirements for religious or personal preferences. Health insurance must be carried through all four years for continued enrollment. Students who fail to do so are subject to suspension. Students are responsible for their own personal health costs and insurance coverage if not under the VCOM health insurance policy. If a student is found to be inactive or out of compliance, VCOM will enroll the student in the VCOM student health insurance plan. 6. DURATION OF POLICY The Minimal Health Requirements for Admission and Continued Enrollment Policy is effective as of the date above and shall remain effective until amended or terminated by the President, with approval by the Board of Directors.
