Appendix A Faculty Recruitment Preliminary Interview Form The purpose of the telephone or equivalent interview is to ascertain a candidate’s interests and qualifications for the position. The interview should be scheduled and an information package containing the position job description, the VCOM annual report and information about the area should be mailed and received by the candidate. Candidate Name: _____________________________________________________________________ Position: ____________________________________________________________________________ Record responses to the following questions: 1. How did you find out about the position and why are you a good candidate?

2. Why are you considering leaving your current position?

3. What is your timeframe for relocation?

4. Describe the characteristics of your ideal work environment.

5. Are there any issues that may impact your ability to obtain a state license or medical malpractice insurance? If so, please explain.

6. What questions do you have about VCOM, or the campus community? (If unable to answer, record and inform candidate you will find out and forward information to them.)

7. If appropriate, ask candidate when they might be available for an interview.
