15 Retrospective Single Center Experience with AngioVac Thrombectomy

Background: The AngioVac is a novel drainage system that provides a minimally invasive approach to remove large volume iliocaval deep venous thromboses, as well as subacute thrombotic material. In this study, the authors investigated a single center’s experience with fifty-eight patients undergoing thrombectomy using the AngioVac system. The patients were followed retrospectively to examine preoperative, intraoperative, and postoperative data. Materials and Methods: This is a retrospective analysis of fifty-eight patients who were treated with the AngioVac mechanical thrombectomy device November 2015 to September 2020 at a single center.

Results: Fifty-eight patients underwent AngioVac mechanical thrombectomy. The mean age was 51 years. 53% of the patients were female and 47% of the patients were male. Indications for AngioVac thrombectomy include deep venous thrombosis in the following vascular beds: inferior vena cava (36.2% of patients), the right atrium (46.5% of patients), tricuspid valve vegetations with pulmonary outflow tracts (13.8% of patients), the suprarenal and infrarenal veins (13.8% of patients), the mesenteric veins (3.45% of patients), the iliac veins (29.3% of patients), and the pulmonary arteries (8.6% of patients). Fifty-six out of the fifty-eight total patients survived to discharge. Success measured as partial

or complete resolution of thrombus burden and restoration of venous blood flow was seen in fifty-five out of fifty-eight patients. Conclusions: The AngioVac device is a safe and effective option and is an excellent treatment option when other procedures, such as open surgery, are ineffective or contraindicated. While there are numerous benefits to AngioVac thrombectomy,

more research is needed before large-scale recommendations can be implemented.
