A diagnostic challenge arises when a patient with diffuse abdominal pain presents after a bilateral salpingo-oophorectomy surgery in the setting of both Struma Ovarii tumor and a source of infection. We report a case depicting this dilemma in a 70-year-old female with history of Struma Ovarii and gastric bypass who presented with diffuse abdominal pain. Computed tomography revealed large inflammatory reaction in the right lower quadrant. The likelihood of appendicitis raised, leading to the recommendation to surgically explore the abdomen which converted to a reduction of an internal hernia and removal of appendix. Nevertheless, a high index of suspicion is necessary in this type of clinical scenario.
