Context: Anatomical variations of the left renal vein, such as the type 2 retroaortic left renal vein (RLRV), are uncommon but clinically significant. While often asymptomatic, these variations are crucial to recognize in surgical planning due to potential complications, including increased venous pressure, varicocele, and hematuria. Understanding these variations is particularly important in patients with comorbidities such as atherosclerosis, which can exacerbate the clinical impact. Report of Case: Through cadaveric dissection, a case was discovered involving an 85-year-old male donor with a complicated medical history that includes sick sinus syndrome, chronic obstructive pulmonary disease, and coronary artery disease. Notably, observations revealed a RLRV that crosses posteriorly to the aorta and drains into the inferior vena cava at the lumbar vertebra L4 level, diverging from the typical anterior course. Conclusions: The current case highlights the importance of understanding the presentation, diagnosis, and treatment of anatomical variations. Conservative treatments for RLRV

typically involve close monitoring and symptom management, including observation, pain control, and addressing hematuria without immediate surgical intervention. These approaches align with findings in the literature, where RLRV-related symptoms often resolve with conservative management in some cases, while others may require surgical intervention, such as nephrectomy for nonfunctional kidneys, pyeloplasty for ureteropelvic junction obstruction (UPJO), and varicocelectomy for varicocele. This case emphasizes the call for further understanding of anatomical variations to enhance medical education and practice.
